All ABC Board enforcement agents must reside within one of the counties which they are assigned or no more than 25 miles from their place of duty, unless written permission is granted through the chain of command by the ABC Board. This series consists of Address Forms used by ABC enforcement officers to advise the central office and district supervisors of any change of address so that there will be less difficulty in contacting agents in an emergency. Information on the form includes name, home address and telephone number, office/mailing address and telephone number, social security number, dates of birth and employment, and mailing address for pay check. This form is to be completed in duplicate when an agent is hired or a change of address/telephone number occurs.
